LifeBox is a patient-oriented program that ensures patients' deepest held beliefs and care preferences are communicated to their professional caregivers, captured in their electronic medical record, and shared as they transition from one care setting to another.
The LifeBox program was created and piloted through a collaborative effort of Norwood Hospital, Steward Healthcare and thirteen allied healthcare partners, representing regional home care, rehabilitation facilities, long term care, hospice and community services.

Norwood Hospital and the Schwartz Center are now collaborating to share the LifeBox model with other interested healthcare organizations.
